SAFETY SCORE REVIEW - Internal Processing (INTERNAL ONLY)
An overview of our Safety Score review process
Introduction
Overview
All rules that trigger a SS1 & SS2 are visible in the UI however for some rules a 'request a review’ can be submitted. The reviews will go through a detailed and transparent two step review by two members of operations in line with our ‘Safety Score Review’ standards and governance.
Who will process reviews
The first review will be carried out by vetting superintendents.
The second review will be carried out by Global Ops & Operations Managers.
Which rules can a review be requested for
How to Request a Review
-
Only listed rules above are open to a request being submitted.
-
A review can only be submitted on SS1 & SS2 vessels.
-
Please ensure there is reasonable justification for a review to take place before requesting one.
-
‘Review details’ tab, will show all rules that triggered SS1 or SS2 in red, and therefore require review.
-
The review is very similar to a vet... e.g. if rule passes/time window expires etc the rule will automatically change to passed.
-
Safety Score rules can have "Should Auto Close" which means that if the rule has this set to true then automatically the Rule would be suppressed given a specific event.
-
Example: 2 Det in 24Months, if set to auto close it means that after the time period elapses the rule would automatically be suppressed. If False even if time elapses a review needs to be done.
-
Safety Score Rules also feature "Can Override" When set to true, the reviewer can override the rule outcome and suppress it, if false no one can supress such a rule.
-
Submitted reviews will go into a backlog visible on the task page called ‘REVIEW’.
-
This task list is like other task lists such as vets.
-
Global Ops, Ops Managers & Vetting Supers will have access to this tasks page.
-
Similar to a vet you can see the vessel, IMO, date of submission, Safety Score, requested by, the reviewer and status.
2-step-acceptance
A 4-eyed principle is applied to ensure that it is not just one person supressing a rule but instead such a process has to go through 2 different people following strict standards and governance. This is known as first and second reviews.
Action if you are the first reviewer:
-
Once the status is updated, ensure the review is no longer assigned to you.
-
The review is now ready for the second reviewer to finalise the safety score review.
Action if you are the second reviewer:
The second reviewer must update the status of the review as per the above screenshot.
A complete safety score review
-
Final review page upon acceptable – second review.
-
Note the safety score old vs new.
-
The requestors will also be notified of status change via email (if a review was requested).
Safety Score Review – Audit Trail
-
The details of the review can be found in two places.
-
The Safety Score change page will be visible as an event on the activity timeline – which will take the user to the SS change page to see a record of why the vessel changed from SS1 to SS3.
