INCIDENTS - Creating and closing a Incident (INTERNAL ONLY)

How to create and close an incident

1. Navigate to a vessel, click on the 'more' button (3-dots icon) and select Create incident.

 

2. On the ensuing page fill out the necessary details if known.

 

For additional guidance on incident definition click on the alert

 

3. If the incident details are known fill out the details.

 

4. If the event sequence, or additional details are known fill out the necessary details.

 

5. You have the option to close out a incident at the time of creating it, or creating it and leaving the incident open. If you choose to simultaneously close out the incident at the time of creating it, you will be required to provide a reason for close out and attach any necessary documents.

 

6. You will also be required to fill out the overall assessment of the ship managers response. Once completed, select 'next'.

 

7. On the ensuing page, you will be required to confirm the priority incident details.

 

 

8. You will receive a confirmation screen where you will be informed of the incident's computed severity and whether it will or will not be included in the Safety Score calculation.
